How to Enroll

This form is part of the first phase of how candidate properties are evaluated for Program. In completing this form, you are answering many of the initial questions Program representatives will ask when you seek to enroll.

And because this questionnaire covers a wide range of issues, it also provides you a valuable resource to identifying steps you can take to preserve and protect your groundwater supply – even if you elect to not participate in the program at this time. Your thoughts and advise will help ensure the success of this program.
